This Emergency Access Form, is only to be used in the most urgent circumstances where access is required at very short notice, outside of normal working hours (Monday to Friday, between 0830 and 1730), excluding UK bank holidays .

In acknowledgement, the applicant confirms that the need to access site is to attend to a major service affecting fault, where the site is down or not operational. Non-essential requests will be rejected and directed to the Operator Access Portal. Persistent misuse of this form will result in a user having their login suspended or removed.
